The Self Sufficient Matrix (SSM-CAT) screening tool is a relatively simple to use tool, specially designed to help basic social service professionals identify people with complex social care needs (CSCN).
To use it, simply add a “new diagnosis” from the options menu of the person selected in the SSB file and click on the “SSM-CAT” button. From this same screen, you can also access the tool's user manual, although it is recommended not to use it without having completed the online training.
The central objective of the tool is to identify, therefore, the tool does not replace other existing assessment methods, nor the final criterion of the professional.
The SSM-CAT is based on the description of the person's situation at a specific time in 13 areas or domains: finances, work and education, time use, housing, domestic relationships, mental health, physical health, substance use, basic ADL, instrumental ADL, social network, community participation and legal order.
If what the professional needs is to see the evolution of the situation at different moments of the person's support process in a specific domain, he can use the tool and generate as many reports as he deems necessary. The PDF reports resulting from the screening cannot be modified once saved and will always be available in the “Diagnostics” tab with the description of the “Type of diagnosis” SSM-CAT.
