The Hèstia program treats the Family File as the main documentary work tool for Primary Care professionals: it provides support for direct case work, allows for guidance of the intervention system processes and groups all the information generated in one place.
The file includes identification data (personal files of the people involved), diagnostic data of the situation (demands, problems), action plans (intervention plans) and a timed record of the assessments and actions of the professionals.
It is important to close the file at the end of the care and/or intervention process, or in the event that the person and/or family moves from one municipality to another.
